If you would like to make an appointment, please contact our office by phone or email. Our patient appointment hours are shown below.
For new patients, you can download & print your new patient forms from this website. Look under "Patient Paperwork & Resources"
Appointment Cancellations:
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
A $75 fee will be charged if a patient does not show up for an appointment or provide sufficient notice.
Financial:
Payment is due at the end of your appointment. Payment plans can be discussed upon request.
Our practice accepts cash, check, and major credit cards, Apple Pay, and Google Wallet. We do not accept CareCredit.
We will be more than happy to submit your insurance forms for you, and we accept most major insurance providers.
We accept/are in network with UnitedHealthCare, Blue Cross Blue Shield (BCBS), Aetna, Cigna, Medcost, PHCS, and most Medicare plans.
We are NOT in network with Medishare, Medicaid, BCBS HMO Medicare, or TRICARE*.
Please note that Humana HMO Medicare plans require a referral from your primary care physician in order to be seen by a chiropractor.
Please call our office to verify whether your specific insurance plan is accepted and what your anticipated out of pocket costs will be.
*For patients with TRICARE or VA Healthcare benefits, we are a participating provider with the U.S. Department of Veterans "Care in the Community" (CITC) Program for Chiropractic Treatment.
